• While in narcolepsy-cataplexy an altered sympathetic activity has been described related to orexine deficiency, no data are available in idiopathic hypersomnia.• We examined if alterations of the autonomic system are present in this type of hypersomnia.• In our group a prevalence of the parasympathetic activity was present in wakefulness and sleep that may explain some of vegetative symptoms in this type of central hypersomnia.
