• Sodium oxybate might be useful in RBD patients refractory to conventional therapies.• Controlled studies are needed to confirm sodium oxybate efficacy in RBD patients cohorts.• Polysomnographic recording might not be a sensitive method for confirming therapeutic effects in RBD.• Home-based assessment, together with mandatory patient and bed-partner reports, seem to be sensitive to clinical changes.
