Guides are often used during reconstructive operations and those that precede the placement of dental implants.1 When an osteotomy or the repositioning of a bony segment is planned, most guides (bone-bone) are positioned by reference to surrounding bony surfaces.2,3 A large surgical field is required to ensure that guidance is accurate. Teeth can also serve as reference points during guided operations (for example, the insertion of implants). Artefacts around metal objects on computed tomography (CT) render CAD-CAM difficult, but augmented digital images of bone and teeth enable fabrication of surgical osteotomy guides, and repositioning templates permit temporal fixation.
