While reviewing the literature pertaining to the mechanism of fibrosis, with a particular reference to oral submucous fibrosis (OSMF), we found a rather critical association of molecular mediators preceding a fibrotic change (Fig. 1) [1]. OSMF is considered by many as an unsuccessful wound healing process due to persistent injury to the oral mucosa [1–4]. In fibrosis, there is an attempt by the mesenchymal cells to secrete the provisional matrix to restore the tissue barrier function [2–4]. However, persistent activation of mesenchymal cells result in progressive and sustained connective fibrosis thu failing to restore the tissue barrier function [4,5].
